If you would like to speak with someone about an appointment, call H.E.R. Wellness Center at 912-358-2016. Please make certain to complete any and all instructions in time for your appointment.

In an effort to help make your first appointment go smoothly, please bring the following important items with you:

 

  • Valid Photo Identification

  • Proof of Insurance Card

  • Co-payment as noted on your insurance card (cash, check, MC, Visa, Discover)

  • List of all current medications you are taking
